Paperwork Burden Reduction Act Notice to Request Form 1095-C 

In accordance with the Paperwork Burden Reduction Act, Vargo Physical Therapy is informing you that Form 1095-C is available on Paychex Flex and upon request.  The form provides information about health insurance coverage offered to you, and if applicable, your family.   You do not need this form to file your federal tax return.  You should, however, keep it with your tax records for your files.

We will provide your requested form within 30 days of your request or by January 31 of the year after the covered tax year, whichever is later. The form will be mailed unless you specifically consent to receive it electronically.
